Healthfirst and The MediSys Health Network have announced their partnership with Hyphen, a cloud-based platform, to enhance the connectivity of longitudinal data in healthcare. By utilizing Fast Healthcare Interoperability Resource (FHIR) standards, Hyphen enables the seamless integration of member data into provider and hospital electronic health record (EHR) systems. This integration facilitates real-time identification of care gaps, allowing clinicians to address patient needs more effectively. The collaboration aims to create a connected care experience and provide greater insight into patients' health needs, thereby contributing to a more equitable health system. Hyphen's solution eliminates the reliance on spreadsheets by delivering actionable data directly into the workflows of EHRs and other systems, streamlining healthcare providers' operations. It also offers applications for community-based social services providers, allowing for the linkage of medical and community data to provide a comprehensive view of individuals' health and social needs. The ultimate goal of Hyphen is to modernize interoperability, ensuring that providers have access to the right information at the right time to address patients' needs accurately.
